The fluid for an enema is best guided by gravity into the distal colon, so the left Sims position is used. Lying on the left side with the knee drawn up aligns the rectum and sigmoid colon in a way that makes insertion easier and lets the enema solution flow downward into the descending and sigmoid colon and be retained if prescribed.

Choosing a right-side position would send the solution toward the ascending colon rather than the distal segments. A supine position lacks the gravity-assisted path and can make distribution unreliable and increase leakage. Trendelenburg is not used for this procedure because it is uncomfortable, can cause excessive abdominal pressure, and does not improve enema effectiveness.
